We offer many services that your local urgent care, primary care or family doctor’s office offers but with a much shorter wait and results in less time. We are not an urgent care, but a well patient care center. Crescent is also a WOUND CARE CENTER specializing in slow healing and complex wounds. 

If you think you have the flu or covid, we will see you, test you and make recommendations for you. We even do curbside, parking lot testing while you remain in your car if you are too sick to come inside. Just like that, in and out! Men’s health, women’s health screening, health and our very own Wellness Snap Shots which includes concierge lab work without a doctor’s prescription. We will refer you to the providers that can best treat the conditions we find on your Wellness Snap Shot and teach you about BP, stress management, and review your labs with you. Medical weight loss is not just a passing fad or gimmick. GLP and GIP injections are medications used to treat diabetes for years and is now prescribed to aide in weight loss when paired with a nutrient rich diet and exercise. Our goal is long term success in managing obesity.  Medically managing obesity can delay or prevent the onset of diabetes and its complications like amputation, blindness, kidney disease and decrease risk of heart attack, renal failure, stroke, and cancer. Our wound care expertise and experience can be vouched for and each provider has healed multiple wounds in our time as wound care nurse practitioners. Crescent Health Partners, LLC has on staff, full-time Certified DOT Examiners to perform thorough, high quality DOT Exams (same day if all documentation is brought with you). We will send some of the forms for you fill out electronically to make your exam and visit go even faster. We are board-certified nurse practitioners with years of experience in patient care and safety procedures. Your health and the safety of those on the road with you are our top priority.  Our prices are very reasonable, and our visits are faster than your local clinics and urgent care clinics.  We care about your health on and off the road. We also do DOT (legal chain of custody) and NON-DOT drug screening in office daily.

TB Skin tests are planted just under the skin and the reading is completed in 48-72 hours by a nurse or doctor. Some employers require a lab draw for TB blood test, called a QuantiFERON, which is available at our clinic for send out to an accredited laboratory. If either test is positive, you will be sent for a chest xray to ensure you do not have active TB and referred to a specialist for treatment and isolation until you are no longer contagious. At CHP, we perform same day, physical exams for multiple reasons, including sports physicals for adolescents 13 years old up to college aged adults.  

Our visits are thorough and faster than local urgent care and primary care. Prices for 13-17 year old student sports physicals is only $55. We travel for groups to schools by appt.  

CHP board-certified family nurse practitioners will see your child today for a sports physical, ages 13 and up. We are kid-friendly and walk–ins welcome. We follow the guidleines of the American Academy of Pediatrics for assessing and making recommendations for your child to have a winning sports season while starting and staying safe! When you need to really know if you’re pregnant and have already done a home pregnancy test (or not), we can do two types of pregnancy testing including blood work that we send to an accredited lab for reliable results. Your privacy and dignity are assured. We have a list of resources to choose from to care for you and your unborn child.

Paternity testing services in office / clinic or on location at courthouse area available for a fee. Call for details. Staff and providers ensure specimen collection process is collected using strict compliance.  Please do not bring any items of clothing or personal items of the alleged parent. Bring ID for guardian for child and the legal guardian.  

It’s that time of year again- the triad of respiratory illnesses numbers spike: Covid, Flu A&B and RSV. Well, Crescent Health Partners is here to screen you and direct you where to get treatment or some bedrest. We offer curbside swabbing and testing in the car. Please wear a mask in office during winter months or if you have signs of a respiratory illness.  

IV hydration therapy consists of IV infusions of IV fluids with vitamins and minerals to replenish what you’ve lost or are lacking. Crescent Health Partners has a physician-supervised, wound care program located at 212 Broad St. in Sumter, SC. Mobile wound care services for hospice, in-home, or nursing facility wound care. Wound care can by very complex at times and at CHP we employ the team approach to solving wound care issues. We use local specialists to help diagnose reasons and barriers to wound healing and refer patients to the nearest hyperbaric oxygen therapy chambers to help aid in the healing process. We are local community partners in wound healing in the Sumter and surrounding communities like Manning, Bishopville, and Lee Counties. There are many advanced treatments like stem cells and biologics we are able to order when all other treatments stall or are no longer effective. Wound care is evolving and ever changing.
